- Name of your Event
- Date of Event
- Estimated Number of People Attending
- Member in charge of activity
- Will this event have sign-in sheets?
- Description of your activity – (Please include in your description the community groups you hope to reach and your goal for the event)
- Recognition – How do you plan to use the contact information that you collect? How will you follow-up with participants?
- How will you evaluate the effectiveness of the event? (What will make this event a success?)
- Do you plan on having food at this event? What food items? (NOTE-Cost of meals are NOT covered for members or for planning meetings unless they are a part of a community event/meeting). Itemized the cost per item if applicable?
